Application Procedure

Please include all of the following in the application:

1. All enclosed forms must be completed.

2. 75 US registration fee (All late applications must send a $100 registration fee)

3. Passport size photo

4. Two letters of recommendation

5. High School transcripts

All the above should be mailed in one envelope to:
